treatment of post-latissimus dorsi flap breast reconstruction pain with continuous paravertebral nerve blocks: a retrospective review

چکیده
conclusions patients receiving a cpvb in addition to tpvb after ldf reconstruction experienced similar pain to those receiving tpvb alone. a larger, randomized clinical trial is warranted to fully determine the benefits of using cpvb in addition to tpvb for this procedure. results a total of 22 patients were included in this study (11-cpvb and 11-tpvb). the mean nrs pain score of cpvb patients (3.5 (standard deviation (sd) 1.8) was lower than that of the single-injection tpvb patients (4.4 (sd 2.1), however this difference was not statistically significant (p = 0.31). the length of hospital stay and opioid use was not statistically different between groups. objectives the addition of a perioperative continuous paravertebral nerve block (cpvb) to a single-injection thoracic paravertebral nerve block (tpvb) has demonstrated improved analgesia in breast surgery. however, its use following isolated post-mastectomy reconstruction using a latissimus dorsi flap (ldf) has not previously been examined. methods we performed a retrospective review of patients who underwent salvage breast reconstruction with a unilateral ldf by a single surgeon. preoperatively, all patients received a single-injection tpvb with 0.5% ropivacaine. additionally, patients had the option for catheter placement to receive a continuous 0.2% ropivacaine infusion with intermittent boluses. infusions commenced in the recovery room and the catheters were removed on the morning of discharge. the primary endpoint was the mean pain numeric rating scale (nrs) scores for the 24-hour period beginning at 7:00 on post-operative day 1.

Treatment of Post-Latissimus Dorsi Flap Breast Reconstruction Pain With Continuous Paravertebral Nerve Blocks: A Retrospective Review
METHODS We performed a retrospective review of patie...
متن کاملBreast reconstruction using a latissimus dorsi flap after mastectomy.
INTRODUCTION The latissimus dorsi (LD) myocutaneous flap has long been regarded as the second choice flap for autologous breast reconstruction following a mastectomy in our department. Despite uncertainty about donor-site morbidity, it is regarded as a relatively safe procedure; moreover, in contrast to our first choice, the deep inferior epigastric perforator flap, no microsurgical expertise i...
متن کاملFunctional Morbidity Following Latissimus Dorsi Flap Breast Reconstruction
According to the National Institutes of Health, more than 230,000 new cases of breast cancer will be diagnosed in 2014 alone. Following mastectomy, several reconstructive options exist for women with breast cancer. The timing and approach for reconstruction must be addressed rapidly. Although abdominal tissue transfer is described as the preferred method, it may not be best suited to all patien...
متن کاملComparison of morbidity-related seroma formation following conventional latissimus dorsi flap versus muscle-sparing latissimus dorsi flap breast reconstruction
PURPOSE The pedicled, descending-branch muscle-sparing latissimus dorsi (MSLD) flap has been widely used for breast reconstruction following total mastectomy. However, the superiority of the MSLD flap compared to the conventional latissimus dorsi (CLD) flap in preventing seroma formation has not been demonstrated. This study compares the morbidities related to seroma formation following pedicle...
متن کاملCombined TRAM flap with latissimus dorsi myocutaneous flap for reconstruction of a large breast post-radiation induced necrosis
Radiation therapy is a critical component of breast cancer management following breast-conserving surgery. Post-radiation sequelae are greater in women with larger breasts, given the need for higher doses and dosing heterogeneity. The goal of breast reconstruction post-mastectomy is to improve the quality of life and add no more health risk. The optimal reconstruction should make the patient fe...
متن کاملBreast Reconstruction following Breast-conserving Surgery with a Subcutaneous Tissue Expander and Latissimus Dorsi Flap
SUMMARY Corrective surgery following breast-conserving surgery is generally challenging due to severe fibrosis induced by postoperative radiotherapy. Although use of the latissimus dorsi myocutaneous flap offers a safe and reliable option, exposure of the skin paddle to the skin surface is often inevitable to achieve correction of nipple-areola complex malposition, leaving conspicuous, patchwor...
